Clinical Care Associate

Job Summary

A company is looking for a Clinical Care Associate to provide support to Care Managers and assist members with healthcare services.

Key Responsibilities

• Assist with scheduling and coordination of services in the member's Plan of Care
• Facilitate the implementation of services and ensure timely follow-up with members
• Provide telephonic assistance and respond to member and provider inquiries

Required Qualifications

• High School diploma or GED required; Associate's or Bachelor's degree preferred
• Minimum of one year experience in home care, discharge planning, or case management
• Preferred experience in the managed care industry
• Knowledge of care management standards and managed care principles
• Familiarity with electronic medical records and Microsoft Office applications
