Job Summary
A company is looking for a Patient Care Advocate who will work with members and providers to enhance care experiences and close care gaps.
Key Responsibilities
- Acts as a liaison and member advocate between members, families, and providers
- Schedules doctor appointments and arranges transportation for members needing preventive care services
- Conducts outreach and education to members about care gaps and barriers to care
Required Qualifications
- Bachelor's Degree in Healthcare, Public Health, Nursing, Psychology, Social Work, Health Administration, or related field, or equivalent work experience
- 4 years of total work experience in direct patient care, social work, quality improvement, or health coaching, preferably in a managed care environment
- 2+ years of experience in direct patient care, social work, quality improvement, or health coaching
- Preferred certifications include Licensed Practical Nurse (LPN), Licensed Master Social Work (LMSW), Certified Social Worker (C-SW), Licensed Social Worker (LSW), or Licensed Registered Nurse (RN)
