Job Summary

A company is looking for a Telephonic Case Manager RN Medical or Oncology - Remote.

Key Responsibilities

• Make outbound and receive inbound calls to assess members' current health status
• Identify gaps or barriers in treatment plans and provide patient education for self-management
• Coordinate care and make referrals to outside sources while ensuring quality care through communication with members

Required Qualifications

• Current, unrestricted RN license in the state of employment
• Active Compact RN License
• 3+ years of experience in a hospital, acute care, or direct care setting
• Proven ability to type and navigate a Windows-based environment
• Access to high-speed internet and a dedicated work area for privacy
