New York Licensed Care Management Manager

Job Summary

A company is looking for a Manager, Care Management & Training to oversee the care management team and improve performance through training and operational strategies.

Key Responsibilities

• Manage daily operations and performance metrics of the Care Management team
• Train and mentor employees on processes, procedures, and system changes
• Monitor team caseloads and ensure compliance with accreditation and regulatory standards

Required Qualifications

• Bachelor's degree, preferably in Nursing
• Current New York State RN or LCSW license required
• CCM certification preferred
• 5 - 8+ years of relevant clinical work experience
• Experience leading care management teams and working in a health plan environment
