New York State Licensed Care Manager

Job Summary

A company is looking for a Care Manager, 100% Remote.

Key Responsibilities:

• Advocates and educates members on services and self-management techniques
• Conducts assessments to identify barriers and develops care plans in alignment with treatment goals
• Coordinates with healthcare teams and documents member interactions in compliance with procedures

Required Qualifications:

• NYS RN or LCSW/LMSW (any state) for Medical Care Management
• 3 years of experience in mental/behavioral health for Behavioral Health Care Management
• 1 year of pediatric clinical field experience for PEDS positions
• Licensure as LMFT, LMHC, LPC, or licensed psychologist for Behavioral Health roles
• Experience managing large caseloads in a fast-paced environment
