Pennsylvania Licensed Behavioral Health Case Manager

Job Summary

A company is looking for a Behavioral Health Triage Case Manager to work remotely in the Tri-State Area.

Key Responsibilities:

• Provides telephone triage, crisis intervention, and care navigation for at-risk members with mental health and/or substance use concerns
• Engages with members and families to assess needs and provide access to appropriate behavioral health services
• Matches members with suitable providers and coordinates care, ensuring effective communication among all parties involved

Qualifications:

• Active PA Registered Nurse License required; B.S.N. or LSW, LCSW, LPC preferred
• Three to five years of post-licensure behavioral health clinical experience in a healthcare setting
• Experience in Utilization Review or Case Management is preferred
• Knowledge of DSM 5 and ASAM is essential
• Strong computer skills and experience with Microsoft Office Suite
