Registered Nurse Care Manager

Job Summary

A company is looking for a Care Manager Transition of Care.

Key Responsibilities

• Evaluate member needs through post-discharge assessments and medication reconciliation
• Develop care/service plans in collaboration with healthcare providers and interdisciplinary teams
• Facilitate member transitions into active care management and provide educational resources

Required Qualifications

• Master's degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing
• 2 - 4 years of related experience in care management
• Licensure as LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN is required
• Must reside in the Central (CST) or Mountain (MT) Time Zones
• Weekend work is required
