Registered Nurse Case Manager

Job Summary

A company is looking for a Mom and Baby Case Manager - RN.

Key Responsibilities

• Engage members and their support systems through assessments and care planning
• Facilitate inter-disciplinary care team meetings and coordinate care for members
• Educate members about treatment options and community resources to support informed decision-making

Required Qualifications, Training, and Education

• Nursing degree from an accredited program or a Bachelor's degree in a healthcare field
• Minimum of three years of experience in nursing, social work, or a related healthcare profession
• Preferred experience in Medicaid and/or Medicare managed care
• Current unrestricted clinical license as a Registered Nurse, Social Worker, or Professional Clinical Counselor
• Case Management Certification is highly preferred
