Remote Cardiology Medical Assistant

Job Summary

A company is looking for a Chronic Care Management-Medical Assistant or Certified Nurse Assistant (Cardiology).

Key Responsibilities

• Develops patient coaching relationships to support care plans and quality of life
• Communicates abnormal findings to the cardiology care team and monitors patient adherence to care plans
• Acts as a liaison for patients with the practice and providers, escalating changes to reduce hospitalizations

Required Qualifications

• High School diploma or GED
• 1 year of experience in remote chronic care management
• Experience with cardiology patients, specifically heart failure
• Medical Assistant (MA) or Certified Nurse Assistant (CNA) certification
• Experience with EMRs, specifically Athena, NexGen, GEMMS, and e-clinical works
