Job Summary

A company is looking for a Special Programs Care Coordinator (Remote).

Key Responsibilities

• Conduct eligibility screenings for the BHI and GUIDE programs
• Perform outreach to patients and caregivers to educate them on program benefits
• Collaborate with the interdisciplinary team to ensure timely communication and care coordination

Required Qualifications

• High school diploma or equivalent required; bachelor's degree in a related field preferred
• Background or strong interest in social work, behavioral health, dementia care, or care coordination
• Self-starter with strong time management and multitasking abilities
• Proficient in Teams, Microsoft Excel, Word, OneDrive, and remote communication tools
• Willingness to learn about available social service and community-based resources
