Job Summary

A company is looking for a Continuity of Care Registered Nurse to support a diverse member population with education, advocacy, and resource connections.

Key Responsibilities:

• Make outbound calls to assess members' current health status
• Identify gaps or barriers in care and treatment plans and make referrals to outside resources
• Facilitate a continuum of quality care through communication with members and caregivers

Required Qualifications:

• Current, unrestricted RN license in the state of employment
• Ability to work in Pacific Standard Time
• 2+ years of experience in a hospital setting, acute care, or as a telephonic Case Manager
• Demonstrated ability to type and navigate a Windows-based environment
