Job Summary

A company is looking for a Clinical Liaison.

Key Responsibilities

• Manage the immediate referral response process to meet performance goals
• Conduct clinical assessments and evaluate prospective resident referrals based on established criteria
• Communicate determinations effectively with referral partners and community members

Required Qualifications, Training, and Education

• Active and in good standing Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of practice
• Minimum of two years of experience in a similar role within a LTC/SNF/AL/MC setting
• Experience in clinical liaison and admission coordination
• Intermediate to advanced computer skills, including experience with EMR or EHR systems
• Ability to handle confidential information and make independent decisions
