New York RN Care Management Manager

Job Summary

A company is looking for a Manager of Care Management, responsible for overseeing the care management team's operations and performance.

Key Responsibilities

• Manage daily operations and performance metrics of the Care Management team
• Train and mentor team members, conducting evaluations as necessary
• Collaborate with leadership to implement performance improvement strategies and ensure compliance with regulations

Required Qualifications

• Bachelor's degree, preferably in Nursing
• Current New York State RN or LSCSW license required
• CCM certification preferred
• 5 - 8+ years of clinical experience, with health plan experience
• Experience leading care management teams
